The completed and signed registration form (within one year of being tested).
A check for the appropriate membership fee:
Initial membership will be $35 per handler/dog for the first year.
Renewals will be $30 per handler/dog.
Additional: $10 for each addional dog/handler.
Testing fees will remain at $15 for therapy dog testing.
CGC testing will remain $10.
Therapy dog testing & CGC testing will be $20 for the combination.

A cute picture of your dog with his/her face suitable for scanning and printing on his/her ID card.
Pix are returned, unharmed.
The pictures are scanned so we can enlarge or crop out your mother in law, your other dog, etc.
We need your dog’s meds; upon registration and yearly registration renewal we require:
A health certificate or a shot record confirming your dog has an annual physical including:
- A fecal check
- Year round heart worm medication and/or a yearly heartworm test
- Rabies
- Bordatella
- DHLPP
